Memory Support Team offers support, education, and care coordination for LW residents and their care partners.
It provides the following services:
• Brain health education
• Home safety assessments
• Caregiversupport/education
• Medication reviews
• Behavioral symptom assist
• Planning for future needs
• Health care directives
