Memory Support Team, a program of Alzheimer’s OC, offers support, education, and care coordination for those living in dementia and their caretakers in Leisure World community.
Services provided include: • Brainhealtheducation.
• Home safety assessments.
• Caregiver support/ education.
• Medication reviews.
• Behavioral symptom assist.
• Planning for future needs.
• Health care directives.
• Linkage to resources.
