GAF donates $25,000 to Pathways GAF donates $25,000 to Pathways

Pathways was recently awarded a $25,000 donation from the Golden Age Foundation (GAF) to continue providing services to shareholders in need of additional resources and support.

Through their care navigation program, Pathways focuses on empowering residents to find solutions that will help them remain safe and independent in their homes for as long as possible, while ensuring they have a care team helping them along the way. This team includes social workers, nursing students and volunteers.

Pathways also steps in to provide support to residents who receive a life-limiting diagnosis and don’t know where to turn. Pathways will meet with the family and provide education, support and connections to the best hospice centers to meet their needs as well as advocate for the family.

Quarterly, Pathways provide grief support groups on-site at the HealthCare Center as well as short-term individual grief counseling. Over the past few years, Leisure World has experienced much loss and Pathways has given a safe place for residents to be with others and learn how to move forward.

Lastly, Pathways has implemented a volunteer program where Leisure World residents are trained and matched with other residents to assist them and simply be a friend.

All Pathways services are provided at no cost to the family. Residents or family members who are interested in receiving more information on Pathway’s services or to become volunteer can call 562-531-3031.

The Golden Age Foundation has served the Leisure World community for over 50 years. The GAF is a nonprofit 501(c) (3) organization established in 1973 to help enhance the quality of life fro Leisure World residents. Over the years, through the support of volunteers, individuals and organizations the GAF has organized and implemented many programs that have benefitted the community at no cost to residents. The foundation is entirely staffed by fellow resident volunteers.
